Registered Nurse – Primary Care Team (Full/Part‑Time)

About the role

Join a growing interdisciplinary primary‑care team delivering community‑focused health services across Malton, Brampton and Rexdale. As a Registered Nurse employed by WellFort Community Health Services, you will be embedded within partner medical clinics and support high‑quality, equitable care for diverse populations.

Key responsibilities

• Provide comprehensive nursing assessments, health education, and counseling to individuals, families and community groups.
• Deliver chronic disease management, preventive care, immunizations, cancer screening and health promotion services.
• Coordinate care and navigation services, linking patients to broader health and social resources.
• Participate in quality‑improvement initiatives and contribute to program planning and evaluation.
• Support priority populations including newcomers, seniors, racialized communities and those facing barriers to care.

Required profile

• Current registration as a Registered Nurse (RN) with the College of Nurses of Ontario.
• Experience working in primary‑care or community‑health settings.
• Commitment to health equity, cultural responsiveness and serving underserved groups.
• Ability to work collaboratively within an interdisciplinary team.
