Senior CHO Specialist (Senior Case Manager)

About the role

The Senior CHO Specialist provides case management and compliance oversight for Community Homes for Opportunity (CHO) programs within CMHA Durham, supporting clients toward long‑term stability and growth.

Key responsibilities

• Develop individualized client care plans, ensure clients understand services, and build therapeutic relationships.
• Monitor compliance for designated homes, produce reports for the CHO Lead, and manage incident reporting.
• Support internal clinical staff, align resident care plans with recovery‑oriented goals, and maintain the CHO waitlist.
• Serve as primary liaison for homeowners, resolve conflicts, and coordinate community resources.

Required profile

• Post‑secondary degree in mental health, social sciences or related field with at least 5 years of clinical experience.
• Knowledge of Ontario’s Mental Health Act, Health Care Consent Act and related legislation.
• Excellent written and verbal communication, strong critical and creative thinking, and ability to work independently and in teams.
• Valid Ontario driver’s licence, reliable vehicle and satisfactory Vulnerable Sector Check.

Required skills

• Proficiency with Microsoft Office (Word, Excel, PowerPoint, Access, Outlook).
• Basic to intermediate French language skills (asset).
