Nurse Practitioner – NPLC Clinic

About the role

CMHA Durham is an integrated community mental health and primary‑care hub that has served the Durham Region for over 60 years. The Nurse Practitioner will deliver comprehensive health assessments, treatment, and health‑promotion services to individuals, families and the community within the Nurse Practitioner‑Led Clinic.

Key responsibilities

• Conduct comprehensive health and symptom‑directed assessments, synthesize data, and make diagnoses in collaboration with physicians and the health‑care team.
• Order, interpret and follow up on laboratory and imaging tests, prescribe medications within the RN(EC) scope, and provide pharmacological, complementary and counseling interventions.
• Deliver health education, promote disease prevention, and develop individualized care plans with clients and families.
• Document client visits accurately, manage urgent and scheduled appointments, and provide home‑visit services as needed.

Required profile

• Council of Ontario University Program in Nursing Primary Health Care Nurse Practitioner Program Certificate (or recognized equivalent) and a Bachelor of Science in Nursing.
• Master’s degree in nursing or a related health field.
• Current registration with the College of Nurses of Ontario as an RN(EC) and active membership with the RNAO.
• Liability insurance or protection and a satisfactory criminal reference check.

Required skills

• Proficiency with Microsoft Office applications (Word, Excel, PowerPoint, Outlook).
• Experience using internet resources, drug‑database programs and electronic medical‑record systems.
• Intermediate written and verbal French is an asset.
• Valid Ontario driver’s licence for community visits.
