Case Manager – Community Mental Health

About the role

The Canadian Mental Health Association Durham seeks a dedicated Case Manager to support clients in the CREATE program. Reporting to the CREATE & Group Living Lead, you will help clients achieve independence through daily living assistance, case management, and collaborative care planning.

Key responsibilities

• Manage an assigned caseload of mental‑health clients, providing one‑on‑one and group support for life‑skills, independent living, and community integration.
• Develop, implement, monitor, and revise individualized care plans (OCAN, GAIN, Life Skills Assessments) in partnership with clients and the multidisciplinary team.
• Deliver case management services, including medication observation, assistance with ADLs, appointment coordination, and transportation support.
• Maintain safe, clean residential group‑living homes and report maintenance issues to CREATE Management.
• Document client contacts, daily logs, and reports in accordance with CMHA policies.

Required profile

• Post‑secondary education in mental health or related field and at least one year of relevant experience, or an equivalent combination of education and experience.
• Knowledge of Ontario mental‑health legislation (Mental Health Act, Health Care Consent Act, PHIPA) and familiarity with psychotropic medications.
• Strong communication skills, ability to build therapeutic relationships, and capacity to work with diverse populations.
• Flexibility, adaptability, and ability to work both independently and as part of a high‑performance team.

Required skills

• Understanding of mental‑health assessment tools and care‑plan development.
• Ability to observe and document medication administration and side effects.
