Registered Nurse – Case Manager (Home Care)

About the role

Join a supportive rural team as a Registered Nurse Case Manager, overseeing home‑care clients in the County of Flagstaff. You will coordinate care, develop individualized plans, and ensure high‑quality services for community members.

Key responsibilities

• Direct care for clients under the Home Care program.
• Assess client needs and coordinate internal and external resources with an interdisciplinary team.
• Develop, implement, and modify care plans.
• Facilitate client and family education and teaching.
• Make timely referrals and provide direct wound, palliative, and home parenteral therapy as needed.
• Cover additional assignments and be on call as workload dictates.

Required profile

• Current Registered Nurse licence (provisional or full practice permit).
• Experience in wound care, palliative care, and home parenteral therapy.
• Strong assessment, organizational, and leadership abilities.
• Ability to work effectively in a rural, community‑focused environment.
