Pharmacist – Inpatient Care (Toronto Western Hospital)

About the role
The University Health Network is seeking a licensed pharmacist to join the inpatient pharmacy team at Toronto Western Hospital. You will work as an integral member of the patient‑care team, providing comprehensive medication management for admitted patients.
Key responsibilities
- Conduct best possible medication history (BPMH) interviews and perform medication reconciliation.
- Review physicians’ medication orders for appropriateness, safety and evidence‑based practice.
- Identify, resolve and prevent drug‑therapy problems in collaboration with the health‑care team.
- Monitor drug therapy for effectiveness, toxicity and adherence to guidelines.
- Follow up on medication incidents and promote safe medication practices on the unit.
- Respond to drug‑information queries and provide patient education.
- Facilitate discharge medication reconciliation.
- Supervise pharmacy technician staff and support clinical teaching and research activities.
Required profile
- Bachelor of Science in Pharmacy or Doctor of Pharmacy degree (or equivalent).
- Active registration with the Ontario College of Pharmacists (Part A) and certification from the Pharmacy Examination Board of Canada.
- Hospital pharmacy practice experience with a focus on patient‑centered care.
- Excellent verbal and written communication, customer service and interpersonal skills.
- Strong organizational and time‑management abilities.
- Hospital pharmacy residency is an asset.
