About the role
The Patient Navigator (Nurse IV) provides expert guidance and emotional support to individuals and families affected by cancer throughout the entire care continuum. Working within Prairie Mountain Health, the Navigator ensures timely access to clinical services, community resources, and supportive programs, while collaborating with multidisciplinary teams.
Key responsibilities
- Conduct comprehensive assessments of patients’ medical, psychosocial, and logistical needs.
- Develop and implement individualized navigation plans that coordinate appointments, treatments, and community services.
- Serve as a primary point of contact, providing education on cancer diagnoses, treatment options, side effects, and self‑care strategies.
- Facilitate distress management and connect patients and families with counseling, support groups, and palliative resources.
- Build and maintain partnerships with oncologists, primary‑care providers, home‑care agencies, and local community organizations.
- Collect and analyze data from evaluations and quality‑improvement surveys to enhance navigation services.
- Ensure compliance with privacy, cultural competency, and ethical standards in all interactions.
Required profile
- Baccalaureate degree in Nursing.
- Active RN licence with the Manitoba regulatory body.
- Completion of a recognized Oncology Nursing program and current Oncology Nursing certification (CON(C)).
- Completion of an Oncology Patient Navigation course or willingness to complete within 18 months.
- Minimum one year of full‑time patient navigation experience (within the last five years).
- Minimum three years of full‑time experience caring for cancer patients and families (within the last five years).
- Current Basic Cardiac Life Support (BCLS) and CPR certification per Prairie Mountain Health policy.
- Valid Manitoba Class 5 driver’s licence, personal vehicle, and ability to travel within the health region.
Required skills
- Proficiency with Microsoft Outlook, Word, Excel, Access, and PowerPoint.
- Strong competence in Internet applications and general IT tools.
- Experience using Citrix and the Aria Med Onc manager electronic health record system.
