About the role
The Nurse Practitioner will join Kingston Community Health Centres’ integrated team in Napanee, delivering primary and specialty care to equity‑seeking individuals and the broader community. Reporting to the Director of Regional Services and Partnerships, you will work both independently and collaboratively with community physicians across multiple clinic sites.
Key responsibilities
- Provide primary care to patients rostered to community physicians and to individuals without a regular provider, including assessment, diagnosis, prescribing, and ordering diagnostics.
- Design, implement and support specialty clinics such as harm‑reduction, vaccination, screening, and palliative care in partnership with the team.
- Incorporate social determinants of health and cultural safety into client care, especially for Indigenous populations.
- Maintain accurate electronic medical records, participate in team meetings, and contribute to health promotion programs.
- Act as a health advocate, refer to other professionals as needed, and provide home visits when required.
Required profile
- Current registration as a Registered Nurse (Extended Class) with the Ontario College of Nurses.
- Three to five years of nursing experience in primary care or a combination of community, hospital, or public health settings.
- Experience supporting marginalized groups, including Indigenous peoples, clients with addictions, or mental‑health challenges.
- Current criminal and vulnerable‑person check, valid driver’s licence, reliable vehicle and up‑to‑date vaccinations.
Required skills
- Proficiency with Microsoft Office applications.
- Experience using an Electronic Medical Record system (Telus PSS).
