Certified Diabetes Educator – Community Health (Napanee)

About the role

The Certified Diabetes Educator (CDE) will provide community‑based education, prevention, and management services for individuals living with diabetes in Napanee and surrounding areas. Reporting to the Manager of Primary Care, the role can be part‑time or full‑time on a contract basis up to eight months.

Key responsibilities

• Deliver pre‑diabetes, diabetes, and prevention education programs in community settings, including insulin initiation and management.
• Provide home‑visiting services, conduct client screenings, assessments, and referrals in line with Diabetes Canada Clinical Practice Guidelines.
• Collaborate with partner agencies, hospitals, and primary care providers to coordinate care and share information.
• Maintain accurate electronic health records for individual and group encounters and support quality‑improvement initiatives.
• Organize outreach events, diabetes screening activities, and community meetings to promote program services.

Required profile

• Undergraduate degree in foods and nutrition from a recognized university.
• Current registration as a Registered Dietitian with the College of Dietitians of Ontario.
• Minimum three years of community nutrition experience, preferably with diabetes‑focused populations.
• Certified Diabetes Educator (CDE) certification or willingness to obtain it.

Required skills

• Proficiency with computers and various software applications.
• Experience documenting client information in Electronic Health Records (EHR) systems.
