Nurse Practitioner – 0.66 FTE – MD Care

About the role

Community Healthcaring KW is seeking a Nurse Practitioner to join the MD Care Family Health Organization team. This part‑time, 0.66 FTE position (approximately 25 hours per week) provides comprehensive primary care across three community locations in Waterloo Region.

Key responsibilities

• Perform assessments, diagnoses, treatments, health education and counselling in line with College of Nurses of Ontario standards for Nurse Practitioners.
• Apply a Social Determinants of Health perspective to improve individual and community health outcomes.
• Triage client needs via phone or in‑person consultations.
• Maintain accurate, up‑to‑date clinical notes according to professional and organisational policies.
• Participate in the design, implementation and evaluation of health promotion and disease‑prevention programs.
• Work regular and extended hours, including at least one evening per week.
• Attend team, staff and committee meetings to support service delivery and organisational goals.
• Contribute to quality‑improvement initiatives and ensure client safety, privacy and confidentiality.

Required profile

• Current registration as a Nurse Practitioner with the College of Nurses of Ontario.
• Ability to work evenings and occasional weekends.
• Commitment to a part‑time schedule of roughly 25 hours per week.
• Strong collaborative skills to work within an interdisciplinary primary‑care team.
