Registered Social Worker / Psychotherapist (0.6 FTE)

About the role

The Centre for Family Medicine (CFFM) Family Health Team is hiring a Permanent Registered Social Worker or Psychotherapist (0.6 FTE) to expand mental‑health services in the KW4 sub‑region of Kitchener. Reporting to the Manager of the Mental Health program, the clinician will provide direct counselling and case‑management to patients across CFFM partner organisations.

Key responsibilities

• Liaise with physicians and conduct on‑site visits to partner offices to build strong professional relationships.
• Collaborate with physicians and team members to jointly manage patient needs.
• Provide counselling, case management and short‑term therapy to patients referred by family physicians.
• Facilitate mental‑health groups and workshops as required.
• Coordinate with community resource organisations and communicate effectively in written and verbal form, including EMR documentation.
• Apply a variety of theoretical models to assess, treat and manage a wide range of mental‑health issues.
• Undertake additional duties that arise as the role and partnerships develop.

Required profile

• Master’s degree in Social Work or Psychotherapy from a recognized institution.
• Registration with the applicable college and proof of liability insurance.
• Strong experience delivering clinical counselling/therapy, preferably in a community setting.
• Experience or strong interest in working with marginalized populations, addictions, trauma histories and mental‑health diagnoses.
• Excellent problem‑solving, interpersonal, verbal and written communication skills.
• Demonstrated ability to work within interdisciplinary teams and with minimal supervision.
• Minimum of three years relevant experience is considered an asset.
