About the role
The Health Nurse provides holistic, culturally informed care to expectant individuals, infants, and medically fragile children in a community‑based setting. Reporting to the Support Services Supervisor‑Clinical Services, the role supports Indigenous families in York Region, focusing on education, assessments, and supportive interventions.
Key responsibilities
- Conduct 1:1 in‑home visits offering prenatal, postpartum, and feeding support.
- Perform nursing assessments covering nutrition, mental health, substance use, intimate partner violence, and infant readiness.
- Integrate Indigenous knowledge and cultural practices into care plans.
- Collaborate with medical and non‑medical practitioners to create a circle of care and attend case planning meetings.
- Provide caregiver support for infants and toddlers, and assess medically fragile children and youth.
- Prepare medical summaries for adoption or discharge and ensure environmental assessments meet medical protocols.
- Complete required documentation in line with agency standards, CNO regulations, and relevant legislation.
Required profile
- Bachelor of Science in Nursing (BScN) and registration with the College of Nurses of Ontario.
- Minimum three years of nursing experience in hospital, community, or public health settings, preferably with pediatric, emergency, or outpatient exposure.
- Ability to deliver culturally safe care and navigate community resources.
