Full‑Time Pharmacist – Diabetes Care (Brampton, ON)

About the role

LMC Pharmacy is seeking a full‑time Pharmacist to join its Brampton clinic, where pharmacists work in a consultation‑based environment focused on diabetes and chronic disease management. You will collaborate with endocrinologists, diabetes educators, and other health professionals to provide patient‑centered care.

Key responsibilities

• Deliver diabetes counseling, medication therapy management, and comprehensive medication assessments.
• Provide personalized education on medication use, lifestyle strategies, and adherence, both in‑person and over the phone.
• Document consultations in compliance with professional standards and maintain accurate patient records.
• Collaborate with prescribers and allied health providers to ensure coordinated care.
• Support routine pharmacy operations, including dispensing and inventory management.

Required profile

• OCP Part A licensed pharmacist in good standing with current OCP Injection Certification.
• 2–3 years of Ontario pharmacy practice, preferably with diabetes education experience.
• Retail pharmacy background; Certified Diabetes Educator (CDE) preferred.
• Ability to travel locally to other LMC sites and communicate in Punjabi or Hindi is a plus.

Required skills

• Kroll proficiency.
• Experience with electronic medical record (EMR) systems.
