Family Physician – Long Term Care (Part‑time)

About the role

This permanent part‑time position allows you to expand your family practice by joining the long‑term care (LTC) team at South Granville Park Lodge. You will provide comprehensive, resident‑centered care for frail elderly patients in a 107‑bed care home.

Key responsibilities

• Conduct regular proactive visits to residents.
• Perform on‑site medication reviews.
• Attend multidisciplinary care conferences.
• Participate in the 24/7 on‑call program.
• Complete accurate documentation in resident charts.

Required profile

• Board‑certified Family Physician licensed to practice in British Columbia.
• Interest in or experience with long‑term care settings.
• Willingness to commit to the five LTC best‑practice expectations.
• Ability to work collaboratively with an interdisciplinary team.

Required skills

• Proficiency with the PointClickCare electronic medical record system.
