Enrollment Registered Nurse (RN)

About the role

The Enrollment RN serves as the clinical lead for enrolling participants into Habitat Health’s Program of All‑Inclusive Care for the Elderly (PACE). Working closely with Outreach Specialists, Enrollment Specialists, and the Interdisciplinary Team, the RN conducts eligibility and home safety assessments, provides education to prospective participants and caregivers, and ensures a smooth, compliant transition into the program.

Key responsibilities

• Perform comprehensive nursing, eligibility, and home safety assessments in participants’ homes and other care settings.
• Complete Nursing Facility Level of Care (NFLOC) assessments and submit required documentation to meet DHCS regulations.
• Present clinical findings at enrollment eligibility meetings and collaborate with the IDT on enrollment decisions.
• Educate prospective participants and caregivers about the PACE program, addressing clinical questions and fostering trust.
• Maintain accurate clinical documentation and participant records within the electronic health record (EHR) system.

Required profile

• Active Registered Nurse (RN) license.
• Experience conducting home health assessments, preferably with older adult populations.
• Familiarity with DHCS requirements and NFLOC assessment processes.
• Strong interpersonal skills to build relationships with participants, caregivers, and multidisciplinary team members.
