Clinical Pharmacist – Primary Care Network (White Rock)

About the role

Join Lower Mainland Pharmacy Services as a full‑time Clinical Pharmacist at the White Rock Primary Care Clinic. You will work within a Primary Care Network to provide direct patient care, collaborate with prescribers, and contribute to provincial primary‑care initiatives.

Key responsibilities

• Conduct patient assessments, identify drug‑related problems, and develop individualized pharmacy care plans.
• Respond to drug information requests and assist in creating drug‑therapy guidelines.
• Validate prescriptions, resolve medication safety concerns, and document therapeutic decisions.
• Participate in adverse drug reaction and medication‑error reporting programs and implement safety improvements.
• Contribute to evidence‑based pharmacotherapy literature and provide preceptorship to pharmacy students and residents.

Required profile

• Current registration as a Pharmacist in British Columbia.
• Strong clinical background with experience in pharmacotherapy and medication safety.
• Excellent communication skills and ability to work collaboratively with multidisciplinary teams.
• Self‑motivated, adaptable, and committed to continuous process improvement.
