About the role
The Clinical Pharmacist will join the pharmacy department of a hospital, providing medication therapy management across pediatrics, neonatal intensive care, and the family birthing centre. The role combines direct patient care, education of students and staff, and supervision of technical pharmacy personnel.
Key responsibilities
- Verify medication orders electronically and assess patients for pharmaceutical care needs.
- Identify, resolve, and prevent drug‑related problems while providing patient education.
- Perform electronic medication reconciliation and ensure continuity of care.
- Support daily drug distribution, including weekend, evening, and on‑call assignments.
- Teach pharmacy students, interns, and other health‑care professionals; deliver in‑service lectures and clinical presentations.
- Develop, revise, and implement pharmacy standards, protocols, and guidelines.
- Supervise pharmacy assistants, manage investigational, narcotic, and controlled‑drug inventories, and oversee medication room audits.
