Staff Pharmacist – Surgical Wards & Pre‑Admission Clinic

About the role
The University Health Network is seeking a Staff Pharmacist to provide comprehensive pharmacy services to the inpatient surgical wards and the Surgical Pre‑Admission Clinic at Toronto General Hospital. This part‑time, permanent position joins a multidisciplinary patient‑care team focused on safe, evidence‑based medication management.
Key responsibilities
- Conduct best possible medication history (BPMH) interviews and perform medication reconciliation.
- Review and dispense physicians’ medication orders, ensuring appropriateness and compliance with guidelines.
- Identify, resolve, and prevent drug‑therapy problems in collaboration with the health‑care team.
- Monitor drug therapy for effectiveness, toxicity, and adherence to evidence‑based protocols.
- Follow up on medication incidents and promote safe medication practices on the unit.
- Respond to drug‑information queries, provide patient education, and facilitate hospital discharge.
- Supervise pharmacy technician staff and support clinical teaching and research activities.
Required profile
- Bachelor of Science in Pharmacy, Doctor of Pharmacy degree, or equivalent.
- Certificate of competency from the Pharmacy Examination Board of Canada.
- Active registration with the Ontario College of Pharmacists (Part A).
- Hospital pharmacy practice experience with a focus on patient‑centered care; inpatient acute‑care experience required.
- Hospital pharmacy residency is an asset.
- Membership in the Canadian Society of Hospital Pharmacists or other professional societies is a plus.
