About the role
The Resident Wellness Coordinator (RPN) supports the Health and Wellness Manager in delivering and coordinating resident care and wellness services within a Chartwell residence. This position provides professional nursing care, clinical leadership, and acts as a liaison among residents, families, physicians, and external healthcare providers.
Key responsibilities
- Assist in assessing, planning, implementing, and evaluating individualized resident care plans.
- Provide direct nursing care, administer medications, and monitor residents for changes in condition.
- Maintain accurate resident charts, wellness documentation, and health records.
- Communicate resident condition changes to physicians, families, and interdisciplinary team members.
- Support admissions, transfers, discharge planning, and resident assessments.
- Provide day‑to‑day leadership, supervision, and support to Personal Support Workers and wellness team members.
- Assign responsibilities, monitor quality of care, and mentor wellness staff.
- Participate in staff meetings, education sessions, and continuous improvement initiatives.
Required profile
- Registered Practical Nurse (RPN) certification.
- Experience delivering nursing care in a residential or long‑term care setting.
- Strong communication skills to liaise with residents, families, physicians, and external providers.
- Ability to lead and support a multidisciplinary wellness team.
