Remote Patient Care Coordinator

About the role

We are seeking a compassionate Remote Patient Care Coordinator to be the first point of contact for seniors and their families reaching out to Sailor Health. You will guide them through the intake process, match them with appropriate therapists, and ensure they feel supported throughout their mental health journey.

Key responsibilities

• Answer initial patient and caregiver inquiries with warmth, clarity, and professionalism.
• Guide patients through the intake workflow, including documentation and scheduling.
• Match seniors with suitable therapists and follow up to confirm satisfaction.
• Maintain accurate records in our EHR and CRM systems.
• Collaborate with the clinical team to address any barriers to care.

Required profile

• Empathetic communicator who can handle sensitive conversations.
• Highly organized, detail‑oriented, and able to manage multiple cases simultaneously.
• Mission‑aligned with a passion for improving mental health access for older adults.

Required skills

• Google Workspace
• Healthie (or similar EHR platform)
• HubSpot (or similar CRM)
