Remote Care Navigator – Support Seniors' Mental Health

About the role

We are looking for a remote Care Navigator who will be the first voice a patient or their family hears when reaching out to Sailor Health. In this role you will guide older adults and their caregivers through the intake process with warmth, clarity, and professionalism, ensuring they feel supported every step of the way.

Key responsibilities

• Guide seniors and caregivers through the intake process, answering questions and providing emotional reassurance.
• Match patients with appropriate therapists and coordinate the transition into care.
• Maintain accurate records in our EHR and CRM systems, ensuring no detail is missed.
• Collaborate with the clinical team to monitor patient progress and address any concerns.

Required profile

• Empathetic communicator who can speak with kindness and listen actively.
• Highly organized and detail‑oriented, able to juggle multiple patients at different stages.
• Comfortable with technology and quick to learn new software tools.
• Passionate about improving mental health care for older adults.
• Previous experience in healthcare, customer service, or administrative support is a plus.
