About the role

We’re hiring a full‑time remote Care Navigator to be the first voice a patient—or their family—hears when reaching out to Sailor Health. You will guide older adults and their caregivers through the intake process with warmth, clarity, and professionalism, ensuring they feel supported every step of the way.

Key responsibilities

• Guide seniors and their families through the intake process, matching them with the appropriate therapist.
• Serve as the primary emotional point of contact, providing reassurance and clear information throughout the care journey.
• Maintain accurate records and follow‑up to ensure no steps are missed.

Required profile

• Empathetic and clear communicator who can ease difficult conversations.
• Highly organized and detail‑oriented, able to juggle multiple patients.
• Comfortable with technology and quick to learn new tools.
• Passionate about improving mental health care for older adults.
