Registered Practical Nurse – Community Home Care

About the role

Join a leading home‑care provider as a Registered Practical Nurse (RPN) delivering scheduled community visits. You will support patients of all ages in their homes, working alongside RNs for complex cases and helping individuals stay comfortable and independent.

Key responsibilities

• Conduct scheduled home visits and assess patient needs within your scope of practice.
• Provide safe, ethical nursing care and use appropriate assessment tools.
• Develop, execute, and continuously monitor individualized care plans.
• Build rapport with patients, fostering trust and collaborative care.
• Maintain accurate, up‑to‑date patient records for each encounter.

Required profile

• Current registration with the College of Nurses of Ontario (CNO) as an RPN in good standing.
• Valid driver’s licence and reliable vehicle for travel to patient residences.
• Reliability, flexibility, and willingness to travel within a designated area.
• Experience in community or home‑care settings is an asset.
• Ability to provide on‑call coverage, including after‑hours, to ensure patient safety.
