Registered Nurse – Home Care Case Manager

About the role
The Registered Nurse will serve as a Case Manager within the Home Care program, overseeing client care from admission through discharge. You will use strong assessment, organizational, and leadership abilities to coordinate interdisciplinary resources and ensure safe, high‑quality care.
Key responsibilities
- Direct the care of home‑care clients, developing and updating individualized care plans.
- Assess client needs and coordinate internal and external resources with the interdisciplinary team.
- Provide wound care, palliative care, and home parenteral therapy as required.
- Facilitate client and family education and support.
- Manage discharge planning, intake assessments, and supportive‑living case management.
- Make timely referrals to stakeholders and deliver direct care when needed.
- Participate in on‑call rotation and cover additional assignments based on workload.
Required profile
- Completion of an accredited nursing education program.
- Active or eligible registration and practice permit with the appropriate regulatory body.
- Experience in wound care, palliative care, and home parenteral therapy.
- Ability to lead, teach, counsel, and advocate for patients and families.
- Strong critical‑thinking, problem‑solving, and decision‑making skills.

What we offer
- Provisional permit hourly rate $39.58‑$54.16; full licence rate adjusted per UNA guidelines.
- Casual/relief employment with flexible shift patterns (days, evenings, nights, weekends, on‑call).
- Opportunity to work in a multidisciplinary home‑care environment.
- Vehicle and driver’s license required for travel between client sites.
