Registered Nurse – Full‑Scope Primary Care (Permanent, 37.5 hrs/week)

About the role

We are looking for a self‑directed Registered Nurse to join the Peterborough Family Health Team and support the Peterborough Street Medicine program. The RN will provide comprehensive, low‑barrier primary health care to individuals experiencing homelessness or precarious housing, working within a full‑scope practice.

Key responsibilities

• Assess patients in outreach‑based and shelter‑based clinics and determine appropriate nursing interventions.
• Deliver foot care, wound care, and harm‑reduction‑informed treatments.
• Perform phlebotomy, administer injections, and support diagnostic testing.
• Document patient history, concerns, and care in the EMR.
• Administer medications and treatments under medical directives.
• Run RN‑led clinics such as blood pressure management, HPV screening, and wound care.
• Provide case management, system navigation, and connect patients to primary care, specialists, and community resources.
• Conduct outreach with the PSM team, focusing on underserved and unhoused populations.
• Monitor chronic diseases, manage lab results, and ensure timely follow‑up.
• Maintain clinic supplies, medical inventory, and vaccine cold chain.

Required profile

• Registered Nurse (RN) licensed by the College of Nurses of Ontario.
• Commitment to health equity and experience working with vulnerable populations.
• Ability to work independently in a low‑barrier, outreach environment.
• Strong collaboration skills with interdisciplinary teams.
