Registered Nurse – Full-Time Primary Care Team

About the role

Join a growing Primary Care Action Team at Thames Valley Family Health Team, one of Ontario’s largest family health teams. As a Registered Nurse you will work on‑site at the 1051 Wonderland Rd. South clinic in London, Ontario, delivering comprehensive, patient‑centered care within an interdisciplinary environment.

Key responsibilities

• Provide direct nursing care to individual patients and groups, including education programs.
• Conduct thorough clinical assessments and implement medical directives.
• Collaborate with physicians, nurse practitioners, pharmacists, social workers, dietitians, and community resources.
• Utilize EMR and virtual‑care technologies to document care and support remote interactions.
• Participate in chronic disease management and patient self‑management initiatives.

Required profile

• Registered Nurse in good standing with the College of Nurses of Ontario.
• Bachelor of Science in Nursing (preferred).
• Experience practicing to full scope and delivering group education.
• Valid driver’s license, regular vehicle access, and appropriate insurance.
• Ability to thrive in a fast‑paced, team‑based, on‑site environment.

Required skills

• Proficiency with electronic medical record (EMR) systems.
• Experience using virtual care technologies.
