Nurse Practitioner – Long Term Care (London, ON)

About the role

RhynoCare is seeking an experienced Nurse Practitioner (NP) to work in Long Term Care facilities in London, ON. You will be hired and onboarded as an Independent Contractor. Key responsibilities

• Deliver primary health care to LTC residents, managing acute and chronic conditions, health promotion, and disease/injury prevention.
• Provide on‑call coverage as required and perform comprehensive clinical assessments with diagnostic ordering and interpretation.
• Lead clinical teams in admission/discharge, palliative care, pain management, dementia care, and responsive behavior management.
• Collaborate with residents, families, discharge planning services, and community health initiatives to develop person‑centered care plans.
• Participate in quality improvement, research, and education initiatives while dedicating 80% of time to direct resident care.

Required profile

• Registered with the College of Nurses of Ontario (Extended Class) and hold a BScN (MSN preferred).
• Member of the Nurse Practitioners’ Association of Ontario (preferred) and possess professional liability insurance.
• Basic CPR certification, current Vulnerable Sector Check, and required immunizations.
• Ability to manage crises, strong written communication, and flexibility for evenings/weekends.

Required skills

• Crisis management in a healthcare setting.
• Knowledge of RNAO Best Practice guidelines and MLTC Regulations.
