About the role
Join a supportive rural health team as a Registered Nurse Case Manager, overseeing home‑care services for clients in the County of Flagstaff. You will coordinate care, educate families, and ensure high‑quality, patient‑centered services.
Key responsibilities
- Direct the care of clients enrolled in the Home Care program.
- Assess client needs and develop individualized care plans.
- Coordinate with an interdisciplinary team to secure internal and external resources.
- Provide wound care, palliative care, and home parenteral therapy as needed.
- Facilitate client and family education and support.
- Maintain and modify care plans based on ongoing assessments.
- Handle timely referrals and on‑call duties, covering additional assignments when required.
Required profile
- Current Registered Nurse licence (provisional permit accepted).
- Experience in wound care, palliative care, and home parenteral therapy.
- Strong assessment, organizational and leadership abilities.
- Ability to work effectively in a rural community setting.
- Excellent communication skills for client and family teaching.

- Hourly rate $40.76‑$55.79 (adjusted after full licence).
