Registered Dietitian - Long Term Care

About the role

S&R Group is hiring a Registered Dietitian for its long-term care home in Windsor. The role is part-time and combines a clinical function with menu planning and nutrition support for residents and their families.

Key responsibilities

• Maintain a minimum of 30 minutes per resident per month, totaling 70 hours a month
• Monitor nutrition and hydration levels for all residents
• Complete scheduled RAIMDS assessments and assess nutritional status on admission, annually and when status changes significantly
• Prioritize and respond to nutrition referrals in a timely way
• Develop, evaluate and modify nutritional care plans, and liaise with residents, families, substitute decision makers and the interdisciplinary team
• Conduct swallowing assessments as needed, and provide nutrition counselling and education to residents and families
• Take part in care conferences, committee meetings and continuous quality improvement initiatives
• Communicate regularly with administration, physicians, allied health, food services and the Nutrition Manager
• Review and approve seasonal menus and develop individualized menus when needed
• Educate and train dietary and nursing staff

Required profile

• Current registration in good standing with the College of Dietitians of Ontario
• Clear Vulnerable Sector Screen
• At least 2 years of relevant experience in a long-term care home is an asset
• Knowledge of the Fixing Long-Term Care Homes Act, 2007 and the Resident's Bill of Rights

Required skills

• Knowledge of computerized electronic medical records (EMR) programs is an asset
• Swallowing assessment experience is an asset
• Teamwork, strong communication, organization, time management, problem solving and leadership
