Physician – Outreach (Part‑Time, 0.6 or 0.8 FTE)

About the role

Community Healthcaring Kitchener‑Waterloo is seeking a dedicated Physician to join its interdisciplinary outreach team. This part‑time position (0.6 or 0.8 FTE) supports individuals who are homeless or precariously housed, many of whom face complex mental health and addiction challenges.

Key responsibilities

• Provide comprehensive medical assessment, diagnosis, treatment, and health education in accordance with the College of Physicians and Surgeons of Ontario standards.
• Deliver care from a Social Determinants of Health perspective to improve outcomes for individual clients and the broader community.
• Apply a harm‑reduction approach when working with clients experiencing substance use.
• Collaborate with mobile primary‑care teams and participate in HART hub locations.
• Coordinate with partner agencies of the Inner City Health Alliance serving the same client population.

Required profile

• Medical degree and active Ontario physician licence.
• Experience working with homeless, precariously housed, or high‑needs populations.
• Comfort with mental health, addiction, and harm‑reduction care.
• Ability to work within an interdisciplinary team and community‑focused environment.

Required skills
