Pharmacist

About the role

Join Brant Community Healthcare System as a Pharmacist and play a vital role in delivering safe, effective medication therapy within our two‑site community hospital network in Ontario. You will work alongside a multidisciplinary team to support patient care across acute, urgent, and transitional services.

Key responsibilities

• Accurately dispense and verify prescription orders for inpatient and outpatient populations.
• Collaborate with physicians, nurses, and allied health professionals to optimise medication regimens.
• Maintain pharmacy inventory, ensure proper storage, and manage medication shortages.
• Provide patient counseling and education on medication use, side effects, and adherence.
• Participate in medication safety initiatives, adverse drug reaction reporting, and quality improvement projects.
• Ensure compliance with provincial regulations, hospital policies, and professional standards.

Required profile

• Current registration with the College of Pharmacists of Ontario.
• Bachelor of Pharmacy or equivalent degree.
• Strong communication skills and ability to work effectively in a team‑based environment.
• Attention to detail and commitment to patient safety.
• Experience in an acute‑care or hospital pharmacy setting is preferred.
