Pharmacist - Central Operations

About the role

The Central Operations Pharmacist provides inpatient pharmacy support at Toronto General Hospital, working in a fast‑paced environment alongside a multidisciplinary health‑care team. The role focuses on reviewing medication orders, ensuring safe dispensing, and collaborating on patient‑centered care.

Key responsibilities

• Review and dispense physicians' medication orders for appropriateness.
• Identify, resolve, and prevent drug‑related problems.
• Monitor drug therapy for effectiveness, toxicity, and guideline adherence.
• Conduct medication reconciliation and follow up on medication incidents.
• Provide drug information and supervise pharmacy technician staff.
• Perform additional duties consistent with the job classification as required.

Required profile

• Bachelor of Science in Pharmacy, Doctor of Pharmacy degree, or equivalent.
• Certificate of competency from the Pharmacy Examination Board of Canada.
• Active registration with the Ontario College of Pharmacists (Part A).
• Hospital pharmacy practice experience, preferably with patient‑centered care in surgery.
• Preferred: Faculty of Pharmacy appointment, hospital pharmacy residency, active CSHP membership.
