Pharmacist – Central Operations (Inpatient Pharmacy)

About the role
The Central Operations Pharmacist provides centralized inpatient pharmacy support within a fast‑paced, multidisciplinary environment at Toronto General Hospital. You will collaborate closely with clinicians, technicians and other health‑care team members to ensure safe, effective medication use for patients.
Key responsibilities
- Review and dispense all physicians' medication orders for appropriateness.
- Identify, resolve and prevent drug‑related problems.
- Monitor drug therapy for effectiveness, toxicity and adherence to evidence‑based guidelines.
- Conduct medication reconciliation for admitted patients.
- Follow up on medication incidents and promote safe medication practices.
- Respond to drug‑information queries and provide direct or functional supervision of pharmacy technicians.
- Perform additional duties consistent with the job classification as required.
Required profile
- Bachelor of Science in Pharmacy or Doctor of Pharmacy degree (or equivalent).
- Certificate of competency from the Pharmacy Examination Board of Canada.
- Active registration with the Ontario College of Pharmacists (Part A).
- Hospital pharmacy practice experience with patient‑centered care, preferably in surgical settings.
- Strong interpersonal and communication skills, and a team‑oriented attitude.
