Patient Intake Specialist – Mental Health (Remote)

About the role

As a Care Navigator at Sailor Health, you will be the first voice a patient or their family hears when reaching out for mental‑health support. You will guide older adults and caregivers through the intake process with warmth, clarity, and professionalism, ensuring they feel supported every step of the way.

Key responsibilities

• Serve as the primary point of contact for new patients, answering questions and easing concerns.
• Coordinate the intake workflow, matching patients with appropriate therapists and tracking progress.
• Maintain accurate records in EHR and CRM systems, ensuring no detail is missed.
• Provide emotional guidance and reassurance during vulnerable moments.

Required profile

• Empathetic communicator who can speak kindly and listen actively.
• Highly organized, able to juggle multiple patients at different stages.
• Mission‑aligned with a passion for improving mental health care for seniors.
• Open to learning and growing, experience in healthcare or customer service is a plus.

Required skills

• Proficiency with Google Workspace.
• Experience using EHR platforms such as Healthie.
• Familiarity with CRM tools like HubSpot.
