Licensed Practical Nurse – Home Care (Casual/Relief)

About the role

As a Licensed Practical Nurse you will join the Home Care team, reporting to the Home Care Manager and collaborating with a Registered Nurse Case Manager. You will assess, plan, monitor and evaluate clients in their homes or alternate care settings, delivering safe, quality, patient‑centered care.

Key responsibilities

• Conduct comprehensive assessments of clients and develop individualized care plans.
• Coordinate and provide direct nursing care in partnership with clients, families and the interprofessional team.
• Monitor client progress, adjust care plans and document outcomes.
• Ensure compliance with Alberta Health Services policies, standards and service authorizations.
• Travel to client residences, navigating varied environments and physical demands.

Required profile

• Completion of an accredited practical nurse education program.
• Active or eligible registration with the College of Licensed Practical Nurses of Alberta.
• Current Basic Cardiac Life Support – Health Care Provider certification.
• Minimum two years recent home‑care nursing experience.
• Excellent attendance record and reliable references.
• Physical ability to perform duties such as climbing stairs, lifting and walking.

Required skills

• Case management in a community setting.
• Use of Connect Care PCS and service authorization processes.
• Home Parenteral Therapy administration.
• Wound, skin and ostomy care.
• Lower‑limb assessment and palliative support.
• Client education and teaching.
