Licensed Practical Nurse – Community Health (Part‑time)

About the role

Join a supportive, dynamic team delivering community‑based nursing care in the County of Flagstaff. As a Licensed Practical Nurse you will work independently in client homes, providing high‑quality, client‑centered care across all ages.

Key responsibilities

• Assess, plan, and implement nursing care for clients in their homes.
• Supervise and mentor Health Care Aides.
• Collaborate with an interdisciplinary team to develop holistic care plans.
• Provide wound care, palliative care, and client education.
• Document assessments, interventions, and outcomes accurately.

Required profile

• Completion of an accredited practical nurse education program.
• Active or eligible registration and practice permit as a Licensed Practical Nurse.
• Strong assessment, organizational, and leadership abilities.
• Ability to work independently in various weather conditions and lift medical supplies.
