Community Health Nurse – Tri-Cities (BC)

About the role

We are seeking a Community Health Registered Nurse to join our Home Health team in the Tri‑Cities region of British Columbia. The role involves providing independent, client‑focused care in the community, supporting individuals and families to live safely at home.

Key responsibilities

• Establish therapeutic relationships with clients and conduct comprehensive assessments in person or by phone.
• Develop and implement interprofessional care plans for clients with complex chronic conditions.
• Provide education, counseling, and direct clinical care to promote health and prevent hospital admissions.
• Coordinate transitions across the health‑care continuum, linking clients with acute, primary, and community services.
• Collaborate with nurse practitioners, physicians, and other specialists to ensure continuity of care.

Required profile

• Current registration with the British Columbia College of Nurses and Midwives (BCCNM).
• Experience in community or home health nursing, preferably with chronic disease management.
• Strong communication skills and ability to work autonomously within an interprofessional team.
