Community Health Nurse (Registered Nurse)

About the role

The Community Health Nurse (Registered Nurse) works independently in the community setting, delivering comprehensive care to clients with complex chronic conditions. You will join an interprofessional team to support clients and families, helping them live safely at home while coordinating with acute, primary, and community health services.

Key responsibilities

• Establish therapeutic relationships with clients through in‑person or telephone interactions, respecting autonomy, dignity, and privacy.
• Screen referrals, conduct individualized assessments, and develop interprofessional care plans.
• Provide clinical interventions, education, and counseling to manage chronic diseases and promote health.
• Facilitate client transitions across the health‑care continuum using the provincial Primary & Community Care model.
• Collaborate with primary care providers, specialists, families, and community resources to ensure seamless care coordination.
• Assist clients in accessing timely health services and resources to achieve optimal functional levels.

Required profile

• Current registration with the British Columbia College of Nurses and Midwives (BCCNM).
• Experience working with clients who have multiple complex chronic conditions.
• Strong interpersonal and communication skills for client‑centered care.
• Ability to work autonomously and as part of an interprofessional team.
