Clinical Pharmacist (Temporary, up to 9 months)

About the role

The Clinical Pharmacist will join an interdisciplinary acute‑care team at Mount Sinai Hospital on a temporary, up‑to‑nine‑month contract. You will provide direct bedside medication management, support complex patient cases, and contribute to therapeutic decision‑making.

Key responsibilities

• Review and optimize medication regimens for acutely ill patients.
• Participate in multidisciplinary rounds and provide real‑time drug therapy consultations.
• Identify medication‑related risks and recommend evidence‑based adjustments.
• Educate patients and families on medication changes, ensuring clear understanding at discharge.
• Document interventions and collaborate with prescribers, nurses, and other health professionals.

Required profile

• Comprehensive knowledge of pharmacology and therapeutics.
• Strong analytical ability to assess clinical data and anticipate complications.
• Excellent communication skills for translating complex drug information to clinicians and patients.
• Ability to work independently after a tailored orientation.
