Clinical Pharmacist – Outpatient Oncology & Medicine/Surgery Float

About the role

The Clinical Pharmacist joins the pharmacy department to deliver medication therapy management, provide pharmaceutical care, and support education for students and staff. The role covers outpatient oncology (oral chemo and systemic therapy clinics) and medicine/surgery float assignments.

Key responsibilities

• Verify medication orders electronically and assess patients for pharmaceutical care needs.
• Identify, resolve, and prevent drug‑related problems; provide patient education and medication reconciliation.
• Support daily drug distribution, including weekend, evening, and on‑call duties.
• Deliver in‑service lectures, drug information, and clinical presentations to nursing units, medical staff, and committees.
• Precept pharmacy students and interns, and maintain continuing education certification.
• Supervise pharmacy assistants, manage investigational, narcotic, and controlled‑drug inventories, and ensure safe disposal of expired stock.
• Participate in development and implementation of standards, protocols, and guidelines for drug therapy.

Required profile

• Licensed pharmacist with experience in clinical pharmacy practice.
• Current continuing education certification.
• Ability to supervise technical personnel and teach professional students.
