Clinical Pharmacist – Inpatient Surgery/Medical Unit

About the role

Unity Health Toronto is seeking a Clinical Pharmacist to join its inpatient surgery or medical unit, or clinic. The pharmacist will deliver direct patient care, ensuring safe and effective drug therapy while collaborating with multidisciplinary teams.

Key responsibilities

• Review medication orders and assess patients requiring pharmaceutical care.
• Identify, resolve, and prevent drug‑related problems for assigned patients.
• Provide patient education, continuity of care, and drug information.
• Promote accurate, safe, cost‑effective drug use and distribution across the hospital, including weekend, evening, and on‑call rotations.
• Support and participate in pharmacy‑related educational and research programs.
• Contribute to the development, revision, and implementation of standards, protocols, procedures, and guidelines related to drug therapy and pharmacy practice.

Required profile

• Registration with the Ontario College of Pharmacists.
• Hospital Pharmacy Practice Residency or equivalent experience.
• Minimum of one year of clinical pharmacy experience in a teaching hospital, preferably in medical or surgical settings.
• Commitment to student and patient teaching, with strong communication and teamwork abilities.
• Interest in research involvement and alignment with the mission and values of St. Michael’s Hospital.
