Case Manager - Intensive Youth and Family Program

About the role

BGC Okanagan is seeking a Case Manager for our Intensive Youth and Family Program in Salmon Arm, BC. In this role you will support at‑risk youth and their families by building resilience, strengthening life skills, connecting them with community resources, and improving family functioning.

Key responsibilities

• Gather information on youths’ problems, needs and risks through interviews, observations, caregiver meetings and use of inventories, checklists and questionnaires, then review to identify issues.
• Develop and implement family‑centred, short‑term, issue‑specific intervention plans within program guidelines in consultation with a supervisor.
• Plan, prepare and conduct group or individual sessions using active listening, conflict resolution and problem‑solving techniques within a family and natural supports framework.
• Provide skill‑building in communication, relationship‑building, resiliency, self‑esteem, self‑management and systems‑navigation.
• Evaluate the effectiveness of intervention plans, report progress and discuss concerns with the supervisor to resolve problems and achieve objectives.
• Outline program services, provide information and referrals to community service providers, resources and professionals as required.
• Maintain related records and statistics and provide reports to the supervisor.
• Liaise with community service providers, professionals and school personnel, and accompany youth and families to meetings and appointments as needed.

Required profile

• Diploma in a related human/social service field or an equivalent combination of education, training and experience.
• Valid First Aid Certificate.
• Class 5 driver’s licence with a clean driver’s abstract.
• Successful completion of agency screening, including a criminal record check.
