Care Navigator (Advocacy)

About the role

Sailor Health is seeking a remote Advocacy Care Navigator based in Canada to guide existing patients through our advocacy services. You will act as a trusted companion, helping seniors transition from therapy to broader support resources with empathy and precision.

Key responsibilities

• Conduct introductory calls with patients interested in advocacy services, following a structured intake process.
• Guide patients through the advocacy onboarding journey, ensuring clarity and attention to detail.
• Coordinate closely with the advocacy team to facilitate smooth transitions and follow‑up actions.
• Manage patient communications across multiple stages, including follow‑ups, missed calls, and reassignments.
• Maintain accurate records using Google Workspace, EHR platforms (e.g., Healthie), and CRM tools such as HubSpot.

Required profile

• Empathetic communicator who can make patients feel at ease during vulnerable conversations.
• Highly organized and detail‑oriented, able to juggle multiple patients without oversight.
• Comfortable with technology and quick to learn new tools.
• Passionate about improving mental health care for older adults.
• Background in healthcare, customer service, or administrative support is a plus, but not mandatory.
