Visiting Nurse - RPN/RN

About the role

Bloom Care Solutions is seeking a compassionate Visiting Nurse (RPN or RN) to provide high‑quality, person‑centred care to clients in the Mississauga and Halton region. You will work independently in patients' homes, delivering clinical services, conducting assessments, and coordinating care with the broader health‑care team.

Key responsibilities

• Perform initial and ongoing in‑home assessments of health status, mobility, cognition, and safety.
• Administer medications, injections, IV therapy, wound, catheter, and ostomy care.
• Conduct diagnostic procedures such as blood glucose and urine tests and manage equipment like oxygen, feeding tubes, and CPAP.
• Document all assessments, treatments, and observations accurately in the electronic health record.
• Educate patients and caregivers on medication use, wound care, symptom monitoring, and equipment management.
• Coordinate with physicians, pharmacies, and other providers to ensure continuity of care.
• Support discharge planning, arrange community resources, and follow infection‑control, safety, and privacy standards (PIPEDA).

Required profile

• Regulated health professional (RPN) with at least 2 years of experience.
• Current BLS certification and up‑to‑date immunizations.
• Police Criminal Record Check with Vulnerable Sector Screening and TB testing.
• Excellent communication, time‑management, and professionalism.
