Social Work Assistant – Long Term Care (VCH)

About the role

The Social Work Assistant joins Vancouver Coastal Health’s Direct Long Term Care Administration team at Adanac Park Lodge. Working under the Patient Services Manager and clinical guidance from the Practice Coordinator, you will support clients and families during admission and discharge, helping them navigate benefits, housing, transportation and community resources.

Key responsibilities

• Interview clients, families and facilities to gather relevant information.
• Assist clients in accessing external resources and completing applications for benefits, housing, transportation and other services.
• Provide practical support on finances, community resources and maintain up‑to‑date information on agencies.
• Collaborate with Social Workers and interdisciplinary team members in a client‑centred care model.
• Ensure documentation is accurate and complies with organizational policies.

Required profile

• Completion of a recognized Social Service Worker or Social Service Diploma program, or an equivalent combination of education and experience.
• Strong verbal and written communication skills.
• Ability to work effectively with diverse individuals and teams.
• Physical capability to perform job duties.
• Ability to prioritize, organize work and operate related equipment.
• Willingness to undergo a Criminal Records Review Check and report vaccine history as required.
