Remote Nurse Care Navigator

About the role

The Nurse Care Navigator (NCN) provides clinical support within Sentrex’s Patient Support Program. Working remotely, the NCN educates patients, trains them on device use, and offers clinical guidance throughout the treatment journey.

Key responsibilities

• Deliver comprehensive virtual device training to patients and designated caregivers.
• Educate patients on disease state, treatment expectations, medication storage, administration, adherence, and safety.
• Conduct follow‑up calls to assess comfort with injection techniques, address concerns, and ensure continuity of therapy.
• Serve as the primary clinical escalation point for Care Navigators, assessing clinical issues and providing guidance.
• Collaborate with field nurse case managers, clinics, and healthcare providers to maintain seamless continuity of care.
• Adhere to Sentrex policies, procedures, and documentation standards.

Required profile

• Registered Nurse (RN) credential with experience in patient education and device training.
• Strong clinical knowledge of specialty therapies and injection techniques.
• Excellent communication skills for virtual interactions with patients and healthcare teams.
• Ability to work independently in a remote environment while meeting scheduled hours (Mon‑Fri, 10 am‑6 pm EST).
