Registered Nurse – Community Home Care

About the role

This full‑time General Visiting Registered Nurse (RN) position supports patients of all ages in their homes, helping them stay comfortable and independent. You will work a regular morning shift (8 am‑4 pm) with every third weekend off, providing essential community nursing care across a designated area.

Key responsibilities

• Use appropriate assessment tools and techniques to evaluate each patient’s individualized needs.
• Develop, execute, and continuously monitor personalized care plans.
• Engage patients through a collaborative approach, building rapport and trust.
• Maintain and update professional patient records for every encounter and care plan.

Required profile

• Current registration with the College of Nurses of Ontario (CNO) in good standing.
• Valid driver’s licence and reliable vehicle for travel to patient residences.
• Reliability, flexibility, and willingness to provide on‑call coverage after hours.
• Passion for making a difference in the community and ability to travel within the assigned area.
