Registered Nurse – Community Home Care

About the role

Join ParaMed Home Health Care as a Registered Nurse providing community nursing services. You will conduct scheduled home visits, assess patients of all ages, and help them stay comfortable and safe in their own homes.

Key responsibilities

• Use appropriate assessment tools to evaluate each patient’s individualized needs.
• Develop, execute, and continuously monitor personalized care plans.
• Build rapport and trust with patients through a collaborative, patient‑centered approach.
• Maintain accurate and up‑to‑date professional records of all patient encounters and care plans.

Required profile

• Current registration with the College of Nurses of Ontario (CNO) in good standing.
• Reliability, flexibility, and willingness to travel within a designated area.
• Passion for making a difference in the community and ability to share on‑call coverage, including after‑hours.
• Experience delivering community care is an asset but not mandatory.
