Registered Nurse – Community Care

About the role

Join ParaMed Home Health Care as a Registered Nurse (RN) delivering community nursing services across Canada. You will conduct scheduled home visits, providing personalized care to patients of all ages and helping them stay comfortable at home.

Key responsibilities

• Use appropriate assessment tools and techniques to evaluate each patient’s individualized needs.
• Develop, execute, and continuously monitor personalized care plans.
• Engage patients through a collaborative, trust‑building approach.
• Maintain and update professional patient records related to encounters and care plans.

Required profile

• Current registration with the CNO as a Registered Nurse in good standing.
• Valid driver’s licence and access to a vehicle.
• Reliability, flexibility, and willingness to travel within a designated area.
• Passion for community impact and ability to provide on‑call coverage after hours.
