Registered Nurse – Black Health & Wellness

About the role

The Registered Nurse (RN) will join an interprofessional care team to deliver comprehensive, client‑centered care with a focus on mental health promotion, prevention, and culturally safe services for African, Caribbean and Black (ACB) youths and families. Working in mobile clinic sites and other DCHC locations, the RN will prioritize the health needs of Durham’s priority populations, including newcomers and ACB communities.

Key responsibilities

• Conduct comprehensive health assessments and develop individualized care plans.
• Provide mental‑health promotion, education, and preventive interventions.
• Deliver culturally safe nursing care tailored to ACB youths and families.
• Collaborate with physicians, nurse practitioners, counselors, dietitians, and outreach workers.
• Document care accurately in clinical records and ensure compliance with regulatory standards.
• Participate in population‑health needs assessments and community outreach activities.
• Support continuity of care across mobile and fixed clinic sites.

Required profile

• Current Registered Nurse licence in Ontario.
• Experience in community health, mental‑health nursing, and working with diverse cultural groups.
• Demonstrated commitment to health equity and culturally safe practice.
• Ability to work flexibly across mobile and site‑based settings.
